Information and Incentives in Medical Decision Making : five Essays
Although information plays a central role for decision making in general and information asymmetries are prevalent in healthcare, the effect of information on medical decisions is not clear. More is known about the influence of incentives on physician provision behavior. However, it is important to investigate how specific incentives work in a particular context as the interplay of different motivations may lead to other outcomes than each of the involved incentives applied individually. The five studies in this thesis analyze how information and incentives affect decisions of patients and physicians using laboratory experiments. The results show that, first, information about the past and communicated information affect decisions of subjects in the role of patients. Second, financial incentives and their size are relevant for physician behavior, and, third, communicated information also influences decisions of subjects in the role of physicians.
Obwohl Informationen bei der Entscheidungsfindung im Allgemeinen eine zentrale Rolle spielen und Informationsasymmetrien im Gesundheitswesen weit verbreitet sind, ist die Auswirkung von Informationen auf medizinische Entscheidungen unklar. Der Einfluss von Anreizen auf Behandlungsentscheidungen von Ärzten ist stärker untersucht. Es ist jedoch wichtig, die Wirkung spezifischer Anreize in einem bestimmten Kontext zu analysieren, da das Zusammenspiel verschiedener Motivationen zu anderen Ergebnissen führen kann als jeder der beteiligten Anreize für sich genommen. In den fünf Studien in dieser Dissertation wird anhand von Laborexperimenten untersucht, wie Informationen und Anreize die Entscheidungen von Patienten und Ärzten beeinflussen. Die Ergebnisse zeigen, dass erstens Informationen über die Vergangenheit und kommunizierte Informationen die Entscheidungen der Probanden in der Rolle der Patienten beeinflussen. Zweitens sind finanzielle Anreize und deren Höhe relevant für das Verhalten von Ärzten und drittens beeinflussen kommunizierte Informationen auch die Entscheidungen von Probanden in der Rolle von Ärzten.
