Evaluation of Inhibitors of Apoptosis Proteins as a therapeutic target in Gastrointestinal Stromal Tumors

Gastrointestinale Stromatumoren (GIST), die häufigsten mesenchymalen Tumoren des Magen-Darm-Traktes, sind abhängig von den aktivierten Tyrosinkinasen KIT oder dem Platelet-derived growth factor Rezeptor alpha. Die Therapie mit KIT-Inhibitoren führt in einem Großteil der Patienten zu langanhaltenden Krankheitsstabilisierungen, jedoch kommt es häufig unter der Therapie zur Progression. Diese wird a.e. durch unzureichende Apoptose und Resistenzentwicklung verursacht. Inhibitors-of-Apoptosis-Proteine (IAPs) sind Signalmoleküle, welche durch eine BIR-Domäne charakterisiert sind. Die Überexpression von IAPs korreliert in verschiedenen Tumorentitäten mit einer Resistenz gegenüber medikamenteninduzierter Apoptose. In diesem Projekt wurde die genomische Dysregulation, Expression und antiapoptotische Funktion der IAPs in GIST untersucht und eine funktionelle IAP-Hemmung allein und in Kombination mit KIT-Inhibitoren evaluiert. In einem synthetischen Letalitätsscreening einer kooperierenden Arbeitsgruppe wurden die IAPs survivin (rank 62-92/11.194) und XIAP (rank 106-557/11.194) als essentielle Proteine für das Überleben von GIST Zellen gewertet. Die Cellular IAPs 1 und 2 (cIAPS 1, 2) wurden nicht als essentiell gewertet, sodass sich das Projekt auf XIAP und survivin fokussierte. XIAP und survivin waren in GIST Primärtumoren und Zelllinien exprimiert. Die IAP Genloci (BIRC2 (cIAP1), BIRC3 (cIAP2), BIRC4 (XIAP), BIRC5 (survivin)) zeigten numerische Veränderungen, überwiegend Amplifikationen, in 47% der mittels SNP-Array analysierten Tumoren. Eine Exom-Analyse zeigte eine Mutation im Gen des IAP-Antagonisten SMAC (DIABLO) in einem Patienten mit einer fortgeschrittenen Erkrankung. Die Expression der IAPs XIAP und survivin war teilweise abhängig von einer KIT-Aktivierung und IAPs könnten in die KIT-regulierten Überlebenssignalwege involviert sein. Es wurden biochemische Inhibitoren der IAPs cIAP1/2, XIAP und survivin in GIST Zelllinien getestet. Der Survivin-Transkriptionshemmer YM155 konnte die Expression des survivin-Transkripts und der Proteinexpression hemmen und Apoptose induzieren. Eine Behandlung mit SMAC mimetics TL32711 und LCL161 reduzierte die Proteinexpression von cIAPs und XIAP und induzierte ebenfalls Apoptose. Proapoptotische Effekte der Inhibitoren konnten durch simultane Gabe von KIT-Inhibitoren verstärkt werden. Die Ergebnisse dieses Projektes unterstützen die Kombination von IAP-Antagonisten und KIT-Inhibitoren in GIST. Weiterführende Versuche sollten die funktionelle Rolle einer Überexpression, sowie eines Knockdowns der IAPs in GIST untersuchen und prädiktive Faktoren für den Erfolg einer Therapie mit IAP-Antagonisten evaluieren.

Gastrointestinal stromal tumors (GIST), the most common mesenchymal tumor of the gastrointestinal tract, is strongly dependent on tyrosine kinases KIT or Platelet-derived growth factor receptor α. KIT inhibitors confer long lasting disease stabilization in the majority of patients. However, during KIT-inhibitory treatment a subset of GIST cells evade apoptosis and eventually develop resistance. Inhibitors of Apoptosis Proteins (IAPs) are signaling molecules characterized by BIR domains. IAP overexpression was reported to confer resistance to druginduced apoptosis. In this project, we sought to analyze IAP genomic dysregulation, expression and antiapoptotic function in GIST and to evaluate antiapoptotic effects of functional IAP inhibition in GIST alone and in combination with KIT inhibition. In a synthetic lethality screen conducted by a cooperating group, survivin (rank 62-92/11.194 tested proteins) and XIAP (rank 106-557/11.194) were found to be essential proteins for cell survival. Cellular IAPs 1 and 2 were non-essential, so the work was focused on XIAP and survivin. We observed that protein and mRNA of XIAP (BIRC4) and survivin (BIRC5) were strongly expressed in GIST cell line models and primary tumors. Amplification of IAP gene loci (BIRC2 (cIAP1), BIRC3 (cIAP2), BIRC4 (XIAP), BIRC5 (survivin)) was detected in 47% of GIST studied by SNP arrays. Whole exome analyses revealed a mutation in the IAP antagonist SMAC(DIABLO) in a heavily pretreated patient. Expression of XIAP and survivin protein and mRNA in GIST cell lines was partly dependent on KIT activation and IAPs may be involved in KIT-regulated pro survival signaling. Biochemical Inhibitors of survivin, cIAPs, and XIAP were tested in GIST cell lines. Survivin inhibitor YM155 was able to decrease BIRC5/survivin transcription and protein expression and to induce apoptosis. Treatment with SMAC mimetics TL32711 and LCL161 led to reduced cIAP1 and XIAP expression, and induction of apoptosis. Proapoptotic effects of survivin and SMAC inhibitors were enhanced by simultaneous KIT inhibition. Our findings support the combination of KIT inhibition with IAP antagonists in GIST. Further investigations should evaluate the role of IAP overexpression and knockdown in GIST and should evaluate predictive factors for IAP-inhibitory treatment.
