Effekte des modifizierten therapeutischen Fastens auf Beschwerden, Lebensstil und Selbstwirksamkeit bei Patienten mit chronischen Schmerzsyndromen

Hintergrund: Patienten mit chronischen Schmerzen sind bezüglich ihrer Lebensqualität stark eingeschränkt und bedürfen einer langfristigen Behandlung. Empirisch wird in der klinischen Naturheilkunde häufig das therapeutische Fasten im Sinne einer additiven Schmerztherapie eingesetzt. Ziel: Das Ziel dieser Studie ist zu untersuchen, die Wirksamkeit des Heilfastens bei der Patienten mit chronischen Schmerzen (Fibromyalgie, Migräne etc.) Patienten und Methode: In der vorliegenden Arbeit wurden 50 konsekutive fastende Patienten in Bezug auf die subjektive Beschwerdelinderung und Variablen des Gesundheitsbefindens untersucht und 50 nicht fastenden Kontrollpatienten gegenübergestellt. Der stationäre Behandlungsplan umfasste in beiden Gruppen 2 Wochen, die Fastenperiode 7 Tage sowie jeweils 2 Einleitungs- und Aufbautage. Die Anzahl der Migränepatienten betrug in der Fastengruppe 26 %, in der Kontrollgruppe 30 %, während die Anzahl der Fibromyalgiepatienten in der Fastengruppe 52 % und in der Kontrollgruppe 50 % betrug. Der kleinere Anteil der Patienten hatte Gonarthrose, ein Schulter-Arm Syndrom sowie Bandscheibenvorfälle der HWS und LWS als Behandlungsindikation. Während des stationären Aufenthalts erfolgten in beiden Gruppen vergleichbare Ernährungsberatungen, begleitende tägliche Bewegungstherapien, Entspannungsverfahren sowie die Anwendung von Physiotherapie und weiterer Naturheilverfahren wie z.B. Akupunktur. Ergebnisse: In den Ergebnissen zeigte sich eine deutlichere Schmerzlinderung und eine partielle Verbesserung des psychischen Befindens bei Fastern gegenüber Nicht-Fastern. Fasten hatte keinen Effekt auf die gesundheitsrelevante Lebensstilmodifikation und die Motivation zur Änderung des Gesundheitsverhaltens. Studienlimitierungen der vorliegenden Arbeit sind das nicht-randomisierte Studiendesign und die relativ kleine Studienpopulation, die nur an einem Studienzentrum untersucht wurde. Schlussfolgerung: Um die Wirksamkeit des Heilfastens als therapeutischen Ansatz bei chronischen Schmerzen präziser beurteilen zu können, sollten weitere Studien an größeren Populationen erfolgen. Dabei könnten u.a. randomisierte klinische Studien zur Wirkung des Fastens bei chronischen Schmerzsyndromen und Fibromyalgie sowie bei lebensstilabhängigen Erkrankungen sinnvoll sein.

Background: The qualities of life to those patients who suffer from chronic pains are strongly restricted and they need a long-term treatment. Empirically the therapeutic fasting is frequently used in the complementary medicine as an additional pain-therapy. Aim: The aim of this study is to find out the effectiveness of the fasting cure with the patients who suffer from chronic pains (fibromyalgia migraine etc.) Patients and method: 50 consecutive fasting patients were examined and compared with 50 non-fasting control patients with respect to the subjective complaint alleviation and variables of the health condition. The inpatient treatment plan compassed both groups in 2 weeks which included a 7-day fasting period as well as 2 introduction and construction days. The number of migraine patients in the fasting group was 26%, in the control group was 30% while the number of Fibro-patients was 50% in the fasting group and 52% in the control group. A small amount of the patients suffered from Gonarthrosis, shoulder-arm syndrome as well as prolapsed intervertebral discs of the cervical and lumbar spine as a treatment indication. Comparable nutrition consultations, daily therapeutic exercises, relaxation exercises as well as the application of physiotherapy and further naturopathies treatment such as acupuncture, were carried out in both groups during the stationary stay. Results: A clearly pain relief and a partial improvement in the emotional condition are shown with fasting group towards non-fasting. Fasting did not have any effect on the health relevant lifestyle modification and the motivation for the change of the health behaviour. The not randomized design is the limitations of the present study and the relatively small studies population which was only examined at a study center. Conclusion: To be able to judge the effectiveness of the fasting cure as an therapeutical approach at chronic pains more precisely, further studies should be carried out at greater populations. Inter alia randomized clinical studies for the effect of fasting on chronic pain syndromes and fibromyalgia as well as on lifestyle addicted illnesses are suggested.
