Einfluss mesenchymaler Stromazellen aus dem Knochenmark von Patienten mit akuter myeloischer Leukämie auf die Expansion hämatopoetischer Stamm- und Vorläuferzellen

In der Pathophysiologie der akuten myeloischen Leukämie (AML) kommt es unter anderem durch Veränderungen in der Stammzellnische zu einer gesteigerten Proliferation leukämischer Blasten und einer Suppression der gesunden Hämatopoese. Im Rahmen dieser Arbeit sollte der Einfluss von mesenchymalen Stromazellen (MSZ) aus dem Knochenmark von AML Patienten im Vergleich zu MSZ aus dem Knochenmark gesunder Spender auf gesunde humane hämatopoetische Stamm- und Vorläuferzellen (HSVZ) aus Nabelschnurrestblut untersucht werden. Zunächst wurden MSZ durchflusszytometrisch und mittels Differenzierungsansatz phänotypisch sowie funktionell charakterisiert. Alle generierten MSZ exprimierten charakteristische mesenchymale Oberflächenantigene und konnten in Adipozyten und Osteozyten differenziert werden. In der statistischen Analyse wiesen AML MSZ eine signifikant höhere osteogene Differenzierungsfähigkeit auf als MSZ gesunder Spender. Die expandierten MSZ wurden für 14 Tage mit HSVZ aus Nabelschnurrestblut kokultiviert. Die kultivierten HSVZ wurden im Anschluss durchflusszytometrisch und im CFC Ansatz (colony forming cell assay) quantitativ und qualitativ untersucht. Nach Kokultur mit AML MSZ fanden sich signifikant mehr CD45+, CD34+CD133+ und CD34+CD133- HSVZ als nach Kultur mit MSZ gesunder Spender. Im CFC Ansatz zeigte sich nach allen Kokulturansätzen ein Verlust CD34+CD133+ HSVZ mit erythrozytärem Potenzial und somit multipotenter Progenitoren (MPP). Ein signifikanter Unterschied fand sich hier in der Anzahl der CFU-M Kolonien (colony-forming unit-macrophage), welche nach Kokultur mit AML MSZ signifikant häufiger zu beobachten waren. Abschließend wurden alle erhobenen Daten miteinander korreliert und in der Kohorte der AML MSZ fanden sich negative Korrelationen zwischen der Expression von CD73 und dem Anteil CD34+CD133+ und CD34+CD38- Subpopulationen in den primären durchflusszytometrischen Analysen der AML Knochenmarkproben. Ähnliche Ergebnisse ließen sich für CD146 ermitteln, wobei hier zusätzlich eine negative Korrelation mit der CFC Frequenz der HSVZ nach Kokultur bestand. Zusammenfassend haben AML MSZ einen proliferationsfördernden Effekt auf gesunde HSVZ wobei im Rahmen dieser Arbeit keine signifikante Funktionseinschränkung kokultivierter HSVZ beobachtet werden konnte. In der Zukunft sollte dies anhand weiterer Studien mit größeren Patientenzahlen näher untersucht werden.

In patients with acute myeloid leukemia (AML) changes in the bone marrow (BM) stem cell niche lead to increased proliferation of leukemic blasts and suppression of healthy hematopoiesis. The aim of this study was to investigate and compare the influence of mesenchymal stromal cells (MSCs) from the BM of AML patients with healthy donors on the expansion of healthy hematopoietic stem and progenitor cells (HSPCs). First, MSCs were phenotypically and functionally analyzed by flow cytometry and differentiation assays, respectively. All MSCs expressed characteristic mesenchymal surface antigens. Comparison of AML MSC and MSC from healthy donors revealed no significant phenotypic differences. In functional read-outs, all MSCs realized adipogenic as well as osteogenic potentials. However, AML MSCs showed a significantly greater osteogenic differentiation potential compared to MSCs from healthy donors. Expanded MSCs were cultivated together with umbilical cord blood (UCB) derived HSPCs for 14 days. Progeny were quantitatively and qualitatively analyzed by flow cytometry and CFC assay, respectively. In co-culture with AML MSCs significantly more CD45+, CD34+CD133+ and CD34+CD133- HSPCs were found compared to cultures with MSCs from healthy donors. In CFC assay, all conditions showed a loss of CD34+CD133+ HSPCs with erythroid potential and thus multipotent HSCs/MPPs. Of note, significantly more CFU-M colonies were found in coculture with AML MSCs. Finally, data was correlated. CD73 and CD146 expression on AML MSCs negatively correlated with the presence of CD34+CD133+ and CD34+CD38- subpopulations in the BM of AML patients. In addition, CD146 expression showed a negative correlation with the CFC frequency of expanded HSPC after coculture with AML MSCs. In summary, we here show that AML MSCs promote the proliferation of HSPC in coculture assays. However, the functional analysis using the CFC assay showed no impaired function of cultured HSPCs. Further investigations with increased patient numbers should be made in the future.
