Untersuchungen zur akuten metabolischen Azidose bei herzchirurgischen Patienten und in einem tierexperimentellen Modell

Bei herzchirurgischen Operationen wird intraoperativ häufig eine metabolische Azidose als Störung des Säure-Base-Haushalts beobachtet. Der Grund für dessen Entstehung stellt derzeit eines der höchst kontrovers diskutierten Themen des Gebietes dar. Es besteht jedoch Grund zu der Annahme, dass das extrem schnelle Einbringen der Priming Lösung in die systemische Zirkulation des Patienten bei Inbetriebnahme der HLM sowie die Applikation der kardioplegischen Lösung wenige Minuten später in Gegenwart eines konstanten Kohlenstoffdioxidpartialdrucks maßgeblich zur Entstehung beitragen. Zur Evaluierung, ob eine Dilutionsazidose sowohl existent als auch klinisch relevant ist, wurde eine humane Beobachtungsstudie an Bypass-Patienten durchgeführt. Zur Analyse beteiligter Kompensationsmechanismen im Falle einer akuten metabolischen Azidose wurde ein standardisiertes Tiermodell (Ratte) der normovolämischen Hämodilution genutzt. In die konsekutive Beobachtungsstudie wurden 33 Patienten eingeschlossen. Von diesen 33 Patienten wurden 25 (Dilutionsazidose) bzw. 29 (Aminosäureanalyse) je nach Fragestellung ausgewertet. Die Ergebnisse intraoperativer Blutgasanalysen wurden aus dem Operationsprotokoll übernommen. Plasma intraoperativ gewonnener Blutproben wurde bei 80 °C bis zur weiteren Analyse aufbewahrt. Nach Anlage des Blasenkatheters wurde eine Urinprobe gewonnen. Zum Operationsende wurde aus dem Sammelgefäß des Katheters eine weitere Probe entnommen. Die Tiere erhielten zwei Gefäßkatheter in die rechten Femoralgefäße sowie einen Blasenkatheter. Mittels repetitiver Blutentnahmen von jeweils drei Millilitern und simultanem Ersatz dieses Volumen durch jeweils eine kommerziell erhältliche Volumensubstitutionslösung in Abhängigkeit der Versuchsgruppe wurde der systemische Hämatokrit stufenweise auf 10% reduziert. Dieser Dilutionsphase folgte eine zweieinhalbstündige Nachbeobachtungsphase. Während des gesamten Versuchs wurden zu definierten Zeitpunkten Blutgasanalysen angefertigt. Nach Anlage des Blasenkatheters erfolgte die Entleerung der Blase. Sowohl während der Dilutions- als auch der Nachbeobachtungsphase wurde kontinuierlich Urin gesammelt. Die durchschnittlichen Werte von pH sowie Base Excess (BE) der Patienten fielen nach Beginn des kardiopulmonalen Bypass (CPB) von Normalwerten auf 7,34 bzw. -3,1 mEq/L. Während der restlichen intraoperativen Phase erholten sie sich auf Werte im Referenzbereich. Die intraoperative Laktatkonzentration stieg von einem Basalwert von 0,9 mM auf 1,4 mM während des CPBs, um bei Operationsende 2,5 mM zu erreichen. Patienten erhielten intraoperativ bedarfsadaptiert Natriumbicarbonat, im Durchschnitt 102 mmol, sowie durchschnittlich 1,1 L Priming Lösung und 1,6 L kardioplegische Lösung. Die plasmatische Histidinkonzentration nahm von 71 µM bei Operationsbeginn auf 21000 µM nach Beginn des CPBs zu, um bei Operationsende 8000 µM zu erreichen. Bei Operationsbeginn betrug die ausgeschiedene Menge Histidin 47 µmol/mmol Kreatinin, bei Operationsende 6760 µmol/mmol Kreatinin. Intraoperativ nahm die Ausscheidung von Harnstoff von 9,5 mmol/h auf 10,1 mmol/h zu (vermehrte Ausscheidung von 3 mmol). Der plasmatische BE der Tiere fiel in der Nachbeobachtungsphase von 0,5 mEq/L zu Beginn des Experiments auf -3 mEq/L. Der pH-Wert im Plasma betrug 7,4 bei Versuchsende. Die plasmatische Laktatkonzentration blieb über die Versuchsdauer annähernd konstant bei 1,3 mM. Sowohl während der Dilutions- als auch der Nachbeobachtungsphase überstieg die gemessene Kaliummenge die anhand von Berechnungen theoretisch vorhergesagte um etwa 0,6 mmol bzw. 0,5 mmol. Die im Urin enthaltene Ammoniummenge stieg von einem Wert von 0,02 mmol im Initialurin auf 0,30 mmol sowohl in Dilutions- als auch Nachbeobachtungsphase. Ausgeschiedenes Phosphat betrug initial 0,0005 mmol und in den genannten Intervallen 0,03 mmol bzw. 0,015 mmol. Im Initialurin enthaltenes Kalium betrug 0,04 mmol. Urin der Dilutionsphase enthielt 0,45 mmol, jener der Nachbeobachtungsphase 0,4 mmol. Im Zuge der humanen Beobachtungsstudie konnte bei operativen Eingriffen unter Verwendung eines CPBs die Entstehung einer Dilutionsazidose gezeigt werden, die deutlich, aber überaus transient ist. Daher war es im Rahmen der vorliegenden Arbeit möglich, trotz anhaltender Diskussion in Fachkreisen, die generelle Existenz der Dilutionsazidose in routinemäßig angewendeten klinischen Situationen, wie bei Gebrauch eines CPBs, zu belegen. Die bedarfsadaptierte, aber routinemäßige Verabreichung von Natriumbicarbonat Lösung stellt den Beweis für die klinische Relevanz der Dilutionsazidose dar. Bedingt durch die in der kardioplegischen Lösung enthaltene Histidinkonzentration von 198 mM und dem verabreichten Volumen von 1,6 L wurden 300 mmol Histidin bzw. 900 mmol Stickstoff inkorporiert. Die gesteigerte Harnstoffausscheidung war, gemessen an der Belastung, relativ gering. Demnach war die durch die Stickstoff-Elimination bedingte Beeinflussung des Säure-Base-Haushalts in der intraoperativen Phase gering. Etwa 7% der infundierten Histidinmenge (entsprechend 20 mmol) wurden renal ohne vorherige Metabolisierung ausgeschieden. Die im Rahmen der Erwärmung der kardioplegischen Lösung stattfindende Deprotonierung des im Histidin enthaltenen Imidazolrings sollte zu einer Verstärkung der dilutionsbedingten metabolischen Azidose beigetragen haben. Im tierexperimentellen Versuchsteil zeigte sich die Aktivierung verschiedener körpereigener Kompensationsmechanismen. Da die Versuchstiere nicht intubiert waren, verfügten sie über die Möglichkeit respiratorisch zu kompensieren. Zusätzlich legte die Bilanzierung der plasmatischen Kaliumkonzentration eine Beteiligung intrazellulärer Puffermechanismen nahe. Die Urinanalyse legte für den relativ kurzen Versuchszeitraum von insgesamt fünfeinhalb Stunden eine massive Ammoniumausscheidung sowie eine geringere Phosphatausscheidung offen. Die Sekretion freier Protonen stellte sich unter quantitativen Gesichtspunkten als vernachlässigbar dar. Entgegen der Literatur scheint bereits die initiale Steigerung der Ammoniumausscheidung auszureichen, um eine metabolische Azidose in ihrer Schwere deutlich abzumildern, sodass diesem Kompensationsmechanismus bereits eine nennenswerte Bedeutung in der Behebung akuter und nicht nur chronischer Azidosen beigemessen werden muss. Zusätzlich zeigte sich eine stark gesteigerte Kaliumausscheidung.

In the context of cardiac surgery, an intraoperative metabolic acidosis is commonly observed. Currently, the reason for its development constitutes one of the most controversially discussed topics in acid-base homeostasis. There is evidence, that the fast administration of the priming fluid to the patient’s systemic circulation at the onset of the heart-lung machine as well as the subsequent application of the cardioplegic solution both in the presence of a constant carbon dioxide partial pressure are significantly involved. To evaluate whether dilutional acidosis is generally existent as well as clinically relevant, an observational study was conducted on patients scheduled for elective isolated coronary artery bypass grafting with the concomitant need of cardiopulmonary bypass (CPB). For analysis of involved compensatory mechanisms in the case of acute metabolic acidosis, a standardized animal model (rat) was employed. In total, 33 consecutive patients scheduled for elective isolated coronary artery bypass grafting (CABG) with CPB were enrolled in the prospective observational designed study. Out of these 33 patients, 25 (dilutional acidosis) and 29 (amino acid analysis), respectively, were analyzed, depending on the respective question. The results of blood gas analyses taken intraoperatively were assumed from the patient’s protocol. Plasma of intraoperatively gained blood samples were stored at -80 °C for further analysis. After insertion of the urinary catheter a urine sample was taken. At the end of operative procedures, a second sample was obtained. The animals received two catheters into the right femoral vessels as well as a urinary catheter. Employing repetitive blood withdrawals of three milliliters each, and simultaneous substitution of the withdrawn volume with a commercially available volume replacement solution, systemic hematocrit was gradually lowered to 10%. Subsequent to this phase of hemodilution, an observation phase of 2.5 h followed. During the whole experimental phase, blood gas analyses were made. After insertion of the urinary catheter, the bladder was emptied. Over the phase of dilution as well as the observation phase, urine was continuously gathered. Average values of pH and base excess (BE) of patients fell from normal values to 7.34 and 3.1 mEq/L, respectively, after initiation of cardiopulmonary bypass (CPB). During the remaining time of CPB, they recovered to values within the reference range. Intraoperative lactate concentration increased from 0.9 mM to 1.4 mM during CPB to reach 2.5 mM at the end of operative procedures. Patients received sodium bicarbonate as required, 102 mmol on average, as well as averaged 1.1 L priming solution and 1.6 L cardioplegic solution. Plasma histidine concentration rose from 71 µM at basal conditions to 21000 µM after initiation of CPB to decrease to a value of 8000 µM at the end of operative procedures. At the beginning of the operation, excreted histidine amounted to 47 µmol/mmol creatinine, and at the end 6760 µmol/mmol creatinine. In the course of the operation, urea excretion increased from 9.5 mmol/h to 10.1 mmol/h (additional excretion of 3 mmol). Animals’ plasma BE decreased to -3 mEq/L during the experiment. At the end, plasma pH value was 7.4. Plasma lactate concentration stayed almost constant at 1.3 mM during the experimental phase. Both during the phase of dilution and observation phase, measured potassium content exceeded the predicted amount calculated on the basis of infusion and withdrawal by 0.6 mmol and 0.5 mmol, respectively. The urinary amount of ammonium increased from 0.02 mmol in initial urine to 0.30 mmol both in the phase of dilution and the observation time. Excreted phosphate accounted for 0.0005 mmol under basal conditions and 0.030 mmol and 0.015 mmol in the respective intervals. Initial urine contained 0.04 mmol potassium, urine collected during the phase of dilution 0.45 mmol and during the observation phase 0.40 mmol. Although currently being one of the most controversially discussed topics in acid-base homeostasis with regards to magnitude but also its fundamental existence, the dilutional acidosis’ existence could be proven in certain routine clinical situations, like those with the usage of CPB. Dilutional acidosis was prominent but also transient in its appearance. Moreover, routine administration of sodium bicarbonate proves its clinical relevance. Due to the cardioplegic solution’s histidine concentration of 198 mM the body is confronted with a high amount of nitrogen. The need for its excretion already becomes apparent in the intraoperative period in terms of an increased excretion of urea and ammonium. The rise in excretion was nonetheless relatively small in relation to the nitrogen load, since at first the majority of nitrogen was stored in the form of different amino acids. About 7% of the incorporated histidine (i.e. 20 mmol) were excreted without prior metabolism. Nevertheless, during the phase of operative procedures, the impact of the incorporated nitrogen on systemic acid-base status was minor. Enhanced deprotonation of the histidine’s imidazole ring during warming of the cardioplegic solution occurred. Protons originating from this process should have intensified existing dilutional acidosis. In experimental animals, the activation of endogenous compensatory mechanisms due to induction of metabolic acidosis was obvious. Since the animals were not subjected to mechanical ventilation, they had the chance to compensate through altered respiration. In addition, balancing of plasma potassium concentration suggested intracellular buffering. In the animals’ urine, the excreted amounts of free protons, phosphate as well as ammonium were quantified to determine renal compensatory mechanisms. The analysis revealed a massive ammonium excretion within the relative short experimental period of 5.5 hours, whereas phosphate excretion was small. The amount of free protons was negligible in quantitative terms. In contrast to literature, the initial rise in ammonium excretion seems to be sufficient to mitigate a metabolic acidosis in its severity. Therefore, this compensatory mechanism already possesses considerable importance concerning correction of acute and not only chronic acidosis. Apart from the ammonium excretion, also an enhanced potassium excretion was observed during as well as after normovolemic hemodilution.
